Annual Convention 2023 Liability Waiver & Release

PLEASE READ THESE TERMS & CONDITIONS | LIABILITY WAIVER & RELEASE AGREEMENT (“AGREEMENT”) CAREFULLY.

By clicking the box in your convention registration form, you acknowledge that you (the “participant”) have read this agreement and that you accept and agree to all the terms and conditions of this agreement. If the participant does not agree to these terms and conditions, participant may not attend the event.

FRIENDS: The National Association of Young People Who Stutter (“FRIENDS”) continues to monitor policy and procedures during the COVID-19 pandemic. The safety and well-being of our community is our top priority. We are following the most up to date CDC guidelines regarding best practices aimed at managing the spread of the novel and complex virus.

FRIENDS has put in place preventative measures to reduce the spread of the COVID-19 virus; however, the organization, under no circumstances, is able to guarantee that you or a guest will not become infected with the virus while attending a FRIENDS event.

Adherence to FRIENDS event policy and guidelines will be continuously monitored and enforced by organization personnel. Please review the agreement below. Additionally, your signature will be required as a confirmation of your intent and commitment to abide by policies put in place to protect FRIENDS visitors/guests, volunteers, and staff. At this point, we are asking that convention attendees wear a mask, if this is something they are comfortable with. In addition, attendees should bring masks for themselves and families, in case CDC guidelines change.

Agreement:

  • I declare I am not experiencing or exhibiting any COVID-19 related symptoms as outlined by the federal and state Center for Disease Control and Prevention (CDC) such as fever, dry cough, or shortness of breath.
  • I acknowledge I must follow the safety protocols that have been implemented by FRIENDS.
  • I declare that I have not been recently exposed (in the past 72 hours) to a person with a positive and

    confirmed case if COVID-19.

  • I agree that I will self-monitor for COVID-19 symptoms during the convention and isolate if symptoms are

    present.

By clicking the box of acknowledgement for this event attendee waiver and release of liability form, I understand the risk that I may be exposed to the virus and become affected as a result. I accept sole responsibility for any expense, liability, illness, injury, disability or death related to contracting the virus while attending an event hosted by FRIENDS. I hereby release, covenant not to sue, discharge, and hold harmless FRIENDS from all claims of any kind arising out of COVID-19. I understand and agree that release of liability includes any claims towards FRIENDS, organization personnel, representatives, and students whether a COVID-19 infection occurrence exists before, during, or after attendance and participation at a FRIENDS event.

If event attendee is a minor child under the age of eighteen years, said event attendee’s adult parent(s) or legal guardian(s) shall be jointly and severally liable for the event attendee’s performance of this event attendee waiver and release of liability form and represent and warrant to FRIENDS that they have full authority to enter into this event attendee waiver and release of liability form on behalf of such minor.

I and my adult parent(s) or legal guardian(s) agree to be bound by all terms of this event attendee waiver and release of liability form, as indicated by our signatures below.

BY ATTENDING AND/OR PARTICIPATING IN THE ACTIVITY, YOU ARE DEEMED TO HAVE GIVEN A FULL RELEASE OF LIABILITY TO THE RELEASED PARTIES TO THE FULLEST EXTENT PERMITTED BY LAW.
